Over the last twenty years, we’ve helped more surrogates complete pre-screening and move into fertility clinics than anyone else in Canada. Yet for many intended parents who find their own surrogate, the next steps can feel unclear.

Found your own surrogate? Here’s what to do next.

Once you and your surrogate have spent time getting to know each other, feel you’re a good personality fit, and want to find out whether she may meet your fertility clinic’s criteria, start here:

  1. Have your surrogate complete a Surrogate Profile Form
    Review it together, then prepare it to send to your fertility clinic.
  2. Complete a criminal record check
    Ask your surrogate to complete a criminal record check through an online service, such as InstantRecordCheck.ca.
  3. Request birth records
    Have your surrogate request the medical records from her previous pregnancy and birth/births (at your expense: typically between $75–$350 depending how many providers she requests from)
  4. Get her most recent PAP results
    Ask her to request a copy of her most recent PAP test results.
  5. Send everything to your fertility clinic
    Submit the Surrogate Profile, birth records, PAP results and any other documentation your clinic requests.

Your fertility clinic can then review the information and determine whether your surrogate appears to meet their criteria to move forward to formal screening.
